Insurance Coverage Requirements
It is the policy of Missouri State-West Plains that each student-athlete provide the following items to the athletic trainer prior to participation:
- Medical History Questionnaire
- Parent information sheet
- Copy of Medical Insurance (if applicable)
- Release of personal health information authorization form
All cheer team members are required to carry personal insurance or be covered under their parents insurance during athletic participation. The insurance policy must cover athletic participation.
All sports managers are required to carry personal insurance or be covered under their parents insurance while serving in this capacity for the University. The insurance policy must cover participation in athletic related activities.
The student-athlete must provide notification to the athletic trainer for any changes to a health insurance policy as they occur. Failure to provide proper notification can result in delays in payment, collection notices, credit reports, etc. If a cancelation of a policy occurs without proper notification, all bills incurred during that period will be the responsibility of the student-athlete and/or the student-athletes parent/guardian.
All student-athletes are requested to complete the Student-athlete Insurance Questionnaire at the beginning of each academic year of participation.
